If you cannot see the Rabbi in person, please use this form to sell your chametz. This form covers all chametz located at all residences and offices owned by you during the Passover holiday. The form must be submitted by Tuesday, April 4th.

By entering my printed name I acknowledge that electronic signatures have the same legal effect as ink signatures.

I hereby grant permission to Rabbi Steven Suson to act as my legal agent and to sell all chametz, mixtures and objects containing chametz belonging to me and to my entire family. As my agent, he should arrange for this sale to take effect on Passover Eve, in the morning, in accordance with Jewish law and custom. I have placed my chametz and mixtures containing chametz in specially designated storage at the address(es) provided below.

It is customary to make a donation for Maot Chittim (literally “wheat money”), thereby fulfilling the mitzvah of charitable giving in connection with the Passover festival. This donation is specifically used to assist the community and those in need. No contribution is required to sell your Chametz
